Nothing in these worksheets is a figure we found somewhere. Every number comes from the inputs you enter and the method stated on the page: completeness is the five onboarding items you tick, diligence is a baseline of three checks plus points for data access, site access, single-sourcing and spend, and the score is your own weights applied to your own ratings. Vendor onboarding is everything between deciding to use a supplier and being able to pay them safely. It is usually treated as paperwork and it is really a control: the W-9 that makes a 1099 possible at year end, the certificate of insurance that matters only if something goes wrong, the signed agreement that decides what happens when it does, the bank details that are the single most attacked field in any small business, and a named contact so a problem has somewhere to go. Most businesses collect three of the five, start paying, and never come back to the other two, because nothing in the process ever says the file is incomplete.

Ask for all of it once, before the first invoice
The leverage is highest before you have paid anything and lowest after. A single request listing all five items, sent at the point of agreement, gets a better response than five chases across the following quarter, when the supplier already has the work.
Verify the bank details out of band
Call a number you already had, not one from the email carrying the details, and not one in its signature. This is the whole of the control and it takes two minutes. On the worked example an unverified account puts $3,945 at risk on one payment at 30-day terms.
Score the file rather than filing it
A file is either complete or it is not, and the useful version of that is a number. Four of five items is 80%, and naming the missing one is what gets it collected, because 'onboarding' as a whole is never anybody's next action.

What should a vendor onboarding pack contain?
For a US small business: a W-9, a certificate of insurance with its expiry date, verified bank details, a signed agreement, and a named contact with an escalation. Five items. The free worksheet on this site scores which of them you actually have.
Do we need a certificate of insurance from every supplier?
No, and asking for one from a supplier who sells you stationery wastes goodwill. It matters where the supplier comes on site, touches your customers, or could cause a loss you would be liable for. The diligence worksheet sizes this by what the vendor actually touches.
What happens when a supplier asks us to change its bank details?
Treat it as onboarding again, not as an update. A change request is the most common vendor fraud in existence, and the control is identical: call a number you already held and confirm before anything is paid.
